Hiv Voluntary Counseling and Testing
Cyber-VCT Program The 2010 UNAIDS report on the global AIDS epidemic states that based on the 2009 data, 33. 3 million people are living with HIV worldwide and Sub-Saharan Africa makes up 68% of the global total with 22. 5 million people living with HIV. 1 In an effort to address this epidemic, voluntary counseling and testing (VCT) programs have been implemented in many of rural and remote areas. VCT serves as the gateway to HIV prevention, treatment, care and support by allowing people to learn their HIV status.According to World Health Organization (WHO), knowledge of one’s HIV status benefits the individuals, community, and population through awareness and education. 2 Therefore, there have been great efforts to expand VCT services since inception over 20 years ago. Unfortunately, global participation remains low, especially in the remote areas. 3 Studies show that there are various reasons why people do not participate. Some of the main reasons are stigma, lack of awarene ss, lack of access, cost of the test, and test-related fears. Several VCT models have been implemented to address these concerns but research suggests that further work needs to be done. Organic Health Response (OHR) has piloted an innovative Cyber-VCT Program in Mfangano Island by using technology to attract residents to participate. The Cyber-VCT Program uses access to solar-powered internet as an incentive to attract people to do HIV testing. This renewal process will provide the structure for more longitudinal effects.The ultimate goal of Cyber-VCT Program is to encourage voluntary counseling and testing, facilitate the process of referrals to Ministry of Health and FACES treatment programs, connect those who are HIV positive with locally formed psycho-social support, and improve dissemination of HIV education. It is more comprehensive and effective than community stand-alone VCT centers, mobile VCT, Routine offer of VCT or better known as provider-initiated HIV testing and counseling (PITC), and home-based VCT. These other VCT models have increased uptake but they have limitations that the Cyber-VCT program helps address.Stand-alone VCT ce nters values client initiation – people have to travel and access the services on their own but the stigma associated with it is a huge barrier that limits people from utilizing the services at the centers. The mobile VCT provides services with a mobile team traveling to communities in a van equipped with HIV-testing facilities. The Cyber-VCT program has been pilot tested in Mfangano Island in Suba District Lake Victoria (West Kenya).OHR’s vision is to implement an effective comprehensive model starting with Mfangano Island and expanding it to the entire Lake Victoria through micro-clinic networks in hopes that the model can be implemented in other rural impoverished areas to continue fighting the HIV/AIDS epidemic worldwide. Mfangano Island is the appropriate pilot site because it has a population of 19,000 people with an estimate of over 30% local prevalence of HIV/AIDS. One of the major threats of the HIV/AIDS epidemic in the island is the possible extinction of the indigenous Suba language and way of life.In addition, the neglected people of the island face various health, economic, political, environmental, and social issues. The island has no electricity and there is one road that circles the base of the island. The primary occupation is fishing and subsistence farming. The impoverished people rely on the Nile perch stock that has been the source of many other issues. The industrial Nile perch export fishery has resulted in unsafe â€Å"fish-for-sex†practices and natural resource depletion. Overpopulation, deforestation, and unsustainable farming practices are the cause of these environment al issues. 9 OHR is taking a comprehensive multi-step approach starting with the Cyber-VCT program to address the complex interconnected issues feeding into the HIV/AIDS problem. OHR plans to sustain the program by building a local infrastructure with micro-clinic networks that empowers the communities.The Cyber-VCT program is the first layer that serves as the gateway to these networks. The networks are formed to encourage people to support each other in an organic way and make the program attractive to residents. Interested participants will go through a five-week social solidarity program to receive education and training on tools to address the HIV/AIDS epidemic. The workshops will take place at EKC. The goal is for the program participants to provide continuous support to their communities in micro-clinic teams of 5-25 people.In this way, an infrastructure is established for future innovation, such as organic farm plots, composting bio-gas latrines, solar cooking ovens, youth r adio, and other sustainable projects, to occur. Most importantly, the infrastructure empowers local communities rather than merely looking â€Å"western. †The objective is to develop a mutually beneficial connection between the local community-based organizations and the international health governance systems rather than having the impoverished rural communities depend only on international assistance.Global scalability will depend on proving the effectiveness of the comprehensive model because the model required a great amount of resources. This amount does not include the cost of land because it was donated by a community leader so the estimated cost for implementation at other areas may be higher. In addition, the $339,000 includes mostly upfront cost and further detailing of maintenance cost will need to be considered. Some of the maintenance cost will be related to technological equipment or systems, security, and infrastructure sustainability.A comparative study is necessary to show improvements in VCT uptake and HIV prevention, treatment, care and support, to attract funding for implementation in other remote impoverish areas that are affected by the HIV/AIDS epidemic. Furthermore, each workshop within the sol idarity program will need to be closely evaluated to make improvements or eliminate any ineffective training and education. Beyond funding, the mutual beneficial relationship between the local communities and international organizations will also play an important role in scalability for resource stability.Strong local ownership of the program will also be invaluable. The networks will likely serve to engage residents organically because the micro-teams will consist of residents’ neighbors, friends, family members. Perhaps, to further encourage local ownership and stabilize the infrastructure, it may be effective to consider creating jobs for local residents to maintain the EKC and microclinics networks. Another layer to the program can be to train local residents to take on positions that may be initially be filled by international partners.OHR’s Cyber-VCT program strives to provide an innovation solution to address the HIV/AIDS epidemic in remote impoverished areas b y using technology. The program is more innovative than other VCT models in that people are incentivized to participate and become part of a network that is intended to build itself in an organic way. Furthermore, Cyber-VCT Page 5 of 6 Cyber-VCT Program program uses access to technology as cover and ensures confidentiality to address the people’s concern of being stigmatized, which is on the top of list of the many reasons why VCT participate rate remains low.Another way the program is more innovative is that it has a high chance of sustainability because of OHR’s focus on building a local infrastructure. The added benefit of establishing a local infrastructure is that it allows for further innovations to address other social, environmental, economic, political issues that are connected to the HIV/AIDS problem. In regards to scalability, securing funding by proving the effectiveness of the program, maintaining the mutually beneficial relationship between the local and nternational organizations, encouraging strong local empowerment, and other considerations such as creating local job opportunities for residents can help drive down cost and make the program scalable. Contrasting Sonnets 18 and 116 by William Shakespeare Essay
The two poems I will be comparing and contrasting in this essay are two of William Shakespeare’s most famous sonnets. Sonnets numbered 18, ‘Shall I compare thee†¦Ã¢â‚¬â„¢ and 116, ‘Let me not.’ Both of these poems deal with the subject of love but each poem deals with its subject matter in a slightly different manner. Each also has a different audience and purpose. In the case of ‘Shall I compare thee†¦Ã¢â‚¬â„¢ the audience is meant to be the person Shakespeare is writing the sonnet about. Its purpose is to tell the person it’s written about how the speaker feels about them. In the case of ‘Let me not’ the audience is anyone who wishes to read it. Its purpose is to introduce what love should be like. This makes ‘Shall I compare thee†¦Ã¢â‚¬â„¢ much more personal and realistic as a poem about love. ‘Shall I compare thee†¦Ã¢â‚¬â„¢ seems to deal more with the idea of a lover rather than the idea of a relationship, as ‘Let me not’ does. ‘Shall I compare thee†¦Ã¢â‚¬â„¢ deals with the idea of a perfect lover and the fading beauty of both women and the seasons. ‘Let me not’ is about ideal love in its most perfect and purest form. In ‘Shall I compare thee†¦Ã¢â‚¬â„¢ Shakespeare describes a lover ‘more temperate’ than a summer’s day. Shakespeare asserts the opinion that the beauty of summer is nothing compared to this perfect human being. In the poem Shakespeare deals with the fading beauty of this perfect human being and lover. Shakespeare expresses the sentiment that even though outer beauty fades inner beauty (‘eternal summer’) will not fade. The perfect lover’s beauty will not fade because she has been immortalised in a sonnet. ‘So long live this, (sonnet) and this gives life to thee.’ In ‘Let me not’ Shakespeare vocalises the perfect love that does not alter ‘when it alteration finds.’ It is a ‘marriage of true minds’ two like minded people joined in a relationship built on understanding and trust. In both sonnets Shakespeare uses a lyrical and narrative method to convey his points. Also the tone in each is a loving one, although ‘Let me not’ has a slightly more didactic tone. Shakespeare believes so completely in this perfect love that if he is proved wrong he must take back all his writing. Even goes so far as to say that if it is not true then no man has ever really loved. As both of these poems are sonnets they follow a set form. Three four line stanzas followed by one set of rhyming couplets. This style is known as the Shakespearean sonnet form. Each line in every stanza is of a similar length, and it is similar in style to all of Shakespeare’s sonnets. This gives it a regular and controlled rhythm with enjambment. Especially in ‘Let me not’ on line 2 leading onto line 3. It is orderly in structure, which in my opinion goes very well with the subject matter of the sonnets. ‘Let me not’ describes love as ‘ever-fixed’ and ‘never shaken.’ The only thing to break this rhythm is at the end, the rhyming couplet. Shakespeare uses the couplet to reiterate the point he is making in the last two lines. Both sonnets follow the same rhyme scheme. It is also a very simple rhyme scheme A B B A, C D D C, E F F E, G G. This rhyme scheme is in keeping with many of the other sonnets, following the Shakespearea n sonnet form. There is plenty of imagery in both of these sonnets. In ‘Shall I compare thee†¦Ã¢â‚¬â„¢ Shakespeare uses the image of time and the changing of the seasons to show the coming of old age, or rather the loss of youth and beauty. Shakespeare starts off by getting his speaker to ask a rhetorical question to his lover. Shall I compare thee? He is almost asking his audience if this is the right comparison he is making. In the second line of the first stanza he makes up his mind that this comparison is not nearly good enough. Summer is by far the loveliest season and even that pales in comparison to this perfect lover. The reason for this is simply that ‘sommers lease hath all too short a date.’ Summer will fade away quickly but the perfect lover will stay beautiful for longer. Because Shakespeare describes this lover as being more temperate than summer he is free to explain how. She is not affected by her splendour as summer is affected by its. The magnificence of summer is overwhelmed by the intensity of the sun’s light. In this sonnet Shakespeare is saying how summer is too brief, and so are youth and beauty. ‘Every faire from faire sometimes declines.’ The repetition of the word faire is used to signify it’s two meanings within the context of the poem. The first is its simple meaning, a fair day, clear and beautiful. The secondary meaning is the deeper meaning, a fair woman. This second meaning of faire is used to indicate that beauty will one day fade away too like summer does every year. Shakespeare addresses the idea of fading beauty in the third stanza. ‘But thy eternal summer shall not fade.’ Her internal beauty will not leave her, though her external beauty might. She will not grow old or ugly as she has been immortalised in poetry. She will forever be remembered, as she is in this poem in its ‘eternal lines.’ The rhyming couplet at the end of the sonnet summarises the whole poem and finalises it. The summers sun dims and fades away, but the life and beauty of the subject of this poem will be eternal. In ‘Let me not’ the imagery is mainly concerned with unmoving and unchangeable love. The sonnet describes how true love will not change to fit in with circumstance, nor when face with a uncertain situation will it cease. It is described as not being ‘Time’s fool,’ love is not at the mercy of time nor subject to change. Shakespeare describes it as ‘an ever fixed mark,’ that watches storms yet is unmoving. A love that is like a star guiding people. This love described is like a beacon shinning out to all the lost soul trying to find their way back. Love can only be measure to a small degree, but we still do not fully understand it. I believe that what Shakespeare is trying to say is that love’s worth will forever remain a mystery. In the final two lines Shakespeare, one again, uses the rhyming couplet to summarise the sonnet. He is so sure of this ever-fixed mark that he is willing to stake his writing on it. He is even prepared to claim that if this love does not exist then no one has ever really loved.

The Appeal and Popularity of the Bournemouth Beach
Bournemouth ranks as one of the most popular resorts on the south coast of England. It has much to offer any holiday-maker, as the town can boast a range of attractions and facilities to cater for nearly every taste. Bournemouth also possesses some beautiful beaches, with golden sands and safe bathing which are frequently backed by majestic cliffs and a promenade. The main features The core feature of the Beach is of course the Sea with its seven miles of golden sand. Bournemouth beach has a high standard of bathing safety, as there are ‘KidZone' areas and emergency lifesaving Perry buoys at regular intervals. RNLI Beach Rescue provides 7 day a week lifeguard service from Southbourne to Sandbanks between May and September, which is augmented by 5 voluntary lifesaving clubs at weekends and public holidays. The beach is patrolled by beach wardens over the summer and by full time, first aid trained inspectors throughout the year. During the bathing season 2003 the Environment Agency was monitoring bathing water quality all along the coast on a weekly basis; the Partnership's main interest, of course, is the quality of the bathing water at Bournemouth Pier. The beach is cleaned daily during the summer, weekly out of season. Areas of the beach have been designated glass free and can-recycling bins are provided. Dogs are allowed on parts throughout the year though they are banned from certain areas from May to September. The secondary features Beach Facilities There are many excellent facilities available along Bournemouth's beaches. These include toilets with access for disabled visitors, first aid posts, lost children centres and information offices. Catering facilities like â€Å"Harry Ramsden's World Famous Fish and Chips†(Restaurant and Takeaway), â€Å"KFC†and â€Å"Hot Rocks†serve drinks, hot and cold snacks and meals. Other entertainment features include the Amusements Arcade, where you can find all sorts of gambling and computer games. Thrill seekers can hop on the Simulator ride by the Pier. The leisure simulator uses the sophisticated technology previously developed for the training simulators used by tank drivers and astronauts to provide a new form of entertainment, which is enjoyable, exciting and yet completely safe. The Oceanarium is a fully interactive experience with touch screen games, feeding demonstrations and talks, plasma screen documentaries, walk-through underwater tunnel and exhibits to help you discover more about this fascinating underwater world. It brings you face to face with marine life from the furthest reaches of the globe. The IMAX offers the most advanced and compelling film experience in the world. Seeing a film at an IMAX theatre is the perfect group event – whether it is a birthday party, corporate outing or school trip. Every year more than 70 million people visit an IMAX theatre to see a film and over 96% would recommend it to their friends and family. The Pier Theatre, Bournemouth is a purpose built proscenium theatre completed in 1960. The foyer houses a small confectionery kiosk, box-office and public toilets. Disabled access is possible by arrangement through the box office. A Deaf Loop (induction) System is in operation. A unisex disabled toilet is available. Bournemouth also has 2,000 acres of formal gardens, which are frequently judged as the best in Britain, and serve as an attraction for the older generation of visitors. The present pier, which was built in 1880, incorporates a theatre, restaurants, a disco and amusement arcades, which together form Bournemouth's most popular attraction. Bournemouth has relied upon its reputation to bring in the tourists and therefore it has a very good record: In 1996 it was named as â€Å"The World's Cleanest and Greenest City. It has also received Blue Flag Beach Awards every year since 1990 and since 1999 has been awarded the Green Flag Award. The main problem with seaside resorts is the fact that their custom is seasonal, this can be seen in the newspapers where there are advertisements for cheap rates in the hotels, for example Sun Court Hotel offers à ¯Ã‚ ¿Ã‚ ½30 per person per night at the moment, as it is the end of the season. The way this is combated is by the use of the conference centre, which tries to bring business in almost all year round, for example, the Conservative party conference is always held at the end of the season, as the season is then extended for an extra week. Poole relies on recreation in its harbour for its business. 60,000 new houses are to be built in the area by 2010, as it is doing so well. Lots of visitors go across on the sandbanks ferry to Studland where 95% of visitors go to beach. In Poole Harbour, sailing and windsurfing take place, while conservation also takes place with SSSI's for the dunes, mudflats and mines, which bats have inhabited. Swanage has also suffered a decrease in its custom since the 1970's: a total of 3,200 beds in hotels and guesthouses have now been reduced to a total of 1,900. With a population of 9,170, there are a total of 9355 beds available for visitors, with a quarter of a million visitors capable of being accommodated in the whole year, although the main season is 26 weeks long. Despite a large number of visitors, although decreasing, the average day visitor spends à ¯Ã‚ ¿Ã‚ ½9, which is not enough to keep a community running. Therefore, the total number of beds is still set to decrease, as the only real attractions are the Swanage railway, which is now being used for park and ride to Corfe castle, and Durlston Country Park, which receives 150,000 visitors. Despite not being a National Park and a decreasing popularity of seaside resorts, except among the retired population, Dorset seems to be surviving. Some areas do seem to be â€Å"thriving†, although a question of their future does hang over areas such as Swanage. Dorset has a number of attractions that keep it a popular location for visitors, however, whether it is thriving or not is a big question, as whereas places such as Bournemouth are doing very well, others are not doing so well.
